You must report errors or problems regarding your account within sixty (60) days after the mailing of the first statement which contained the problem or error. When you report an error or problem regarding your account, you should provide the following information:

• Your name and Account number;
• A description of the error or problem and the reason why you think an error or problem occurred;
• The dollar amount of the suspected error or problem.

If you orally report an error or problem regarding your account, we may require you to send your complaint or question to us in writing within ten (10) business days. Upon receipt of the written complaint or question, we will conduct an investigation and notify you of the results within ten (10) business days (twenty business days for new accounts) and will make any necessary adjustments to your account based on the results of our investigation. If it is determined, in our sole discretion, that more time is required to investigate and respond to your problem or question, we may take up to 45 days to complete our investigation. In the event that such additional time is required, we will provisionally credit your account within ten (10) business days (twenty business days for new accounts) for the amount you think is in error so that you will have use of the money during the time it takes us to complete our investigation. A provisional credit is a temporary credit adjustment to your account for the time period required to complete our investigation. If we ask you to report your complaint or question in writing, and we do not receive the writing within ten (10) days, we may remove the provisional credit from your account without further notice. Please note that contacting us by telephone will NOT preserve your rights.

If we determine that there was no error, we will send you a written explanation within three (3) business days after we complete our investigation, and any provisional credit to your account will be reversed. If there are not sufficient funds in your account to cover the amount of the provisional credit, the account will be overdrawn and you will be responsible for immediate payment. You may request copies of the documents that we used in our investigation.
